By any reasonable measure, either percent of GDP or dollars per capita, the US spends more on health care than any other industrialized, modern nation.  We spend at least 50% more.  This places a tremendous drain on our competitiveness.  We&#8217;re also the only country with our system of privately insured and paid for care &#8211; with the resulting patchwork of coverage that leaves too many Americans without access to needed care.
</p>
<p>
Help get US costs aligned with those of our competitors.  Help get US, each and every citizen, access to care.
</p>
<p>
Please vote for the reform bill before you now.

The <a href="http://www.oecd.org/health/healthdata">Organisation for Economic Co-operation and Development</a> (OECD) just released their paper on health care costs for 2009.  You can download a  <a href="http://www.oecd.org/dataoecd/46/2/38980580.pdf">handy PDF summary</a>.  This material should be required reading for anyone asking &#8220;How will we pay for universal health insurance under Obama&#8217;s health care proposal?&#8221;
</p>
<p>
The real question is &#8220;How can we continue to afford paying more of our Gross Domestic Product than any other industrialized country?&#8221;  Consider these points about the 30 member countries of the OECD:
</p>
<ul>
<li>On average countries spend 8.9% of GDP on healthcare </li>
<li>The US spends 16.0% of GDP on healthcare </li>
<li>Second place spender France spends 11.0% of GDP </li>
<li>Among the European Union countries Ireland spends the least, at 7.6% &ndash; less then one half the US level</li>
</ul>
<p>
All of the other countries have a form of single payer.  All spend significantly less of the GDP than the US.  Most have better health (measured by infant mortality and expected lifespand) than the US.
</p>
<p>
We&#8217;re paying more for worse care.  How can we afford to keep doing what we&#8217;ve been doing?
</p>
<p>

As a columnist who regularly dishes out sharp criticism, I try not to question the motives of people with whom I don&#8217;t agree. Today, I&#8217;m going to step over that line.
</p>
<p>
The recent attacks by Republican leaders and their ideological fellow-travelers on the effort to reform the health-care system have been so misleading, so disingenuous, that they could only spring from a cynical effort to gain partisan political advantage. By poisoning the political well, they&#8217;ve given up any pretense of being the loyal opposition. They&#8217;ve become political terrorists, willing to say or do anything to prevent the country from reaching a consensus on one of its most serious domestic problems.
</p>
<p>
There are lots of valid criticisms that can be made against the health reform plans moving through Congress &#8212; I&#8217;ve made a few myself. But there is no credible way to look at what has been proposed by the president or any congressional committee and conclude that these will result in a government takeover of the health-care system. That is a flat-out lie whose only purpose is to scare the public and stop political conversation.
</p>

Health reform is a test of whether this country can function once again as a civil society &#8212; whether we can trust ourselves to embrace the big, important changes that require everyone to give up something in order to make everyone better off. Republican leaders are eager to see us fail that test. We need to show them that no matter how many lies they tell or how many scare tactics they concoct, Americans will come together and get this done.

<p>
That nugget comes to us from <a href="http://www.nytimes.com/2009/07/31/opinion/31krugman.html?_r=1">Paul Krugman at the New York Times</a>.  Such is our public debate on health care reform.  People love an existing government run health care program.  People are adamant that government keep its hands out of their health care.
</p>
<p>
Well, some people feel that way.
</p>
<p>
This combination of attitudes &#8211; a love of Medicare (people on it are more satisfied than people with private insurance) and a distrust or hatred of goverment programs &#8211; has complicated the health care reform debate.
</p>
<p>
Some people would prefer employers to give cash and let employees buy insurance on the open market. That, it seems, is very inefficient in paying for health care.
</p>
<p class="indent">
In the individual insurance market, where people buy insurance directly rather than getting it through their employers, so much money goes into underwriting and other expenses that only around 70 cents of each premium dollar actually goes to care.
</p>
<p>
If you are an insurance company employee that 30 cents on the dollar keeping you employed is a good deal.  [<i>easy for the bank employee to write&hellip;</i>]
</p>
<p>
Krugman concludes with:

<p class="indent">
To the extent we have a working health care system at all right now it’s only because government covers the elderly, while a combination of regulation and tax subsidies makes it possible for many, but not all, nonelderly Americans to get decent private coverage.
</p>
<p class="indent">
Now Mr. Obama basically proposes using additional regulation and subsidies to make decent insurance available to all of us. That’s not radical; it’s as American as, well, Medicare.</p>

Some of the costs of not having a public option are simple to calculate, but immeasurable in value. Infant mortality rates in the United States are 6.37 deaths/1,000 live births.  A sampling of other industrialized nations with public health care finds the United Kingdom at 5.01 deaths / 1,000 live births. Canada at 4.63. France at 3.41. If the United States infant mortality matched that of the United Kingdom, just under 6,000 fewer infants would have died in the United States last year. If we could match France around 13,000 fewer infants would have died.

People with right leaning politics tend to be against the public option due to it being part of big government.  They&#8217;re also against abortion because they&#8217;re &#8220;pro life.&#8221;

The practice of torturing suspected terrorists received fresh blows Friday after a magazine reported that a key al Qaeda suspect offered useful intelligence after receiving sugar-free cookies.

That is from a <a href="http://rawstory.com/blog/2009/05/cookies-al-qaeda/">Raw Story story</a> on a <a href="http://www.time.com/time/magazine/article/0,9171,1901491-1,00.html">Time Magazine story</a> detailing  revelations from former FBI interrogator Ali Soufan.  Prisoner Abu Jandal had been held in Yemeni for over a year yielding no information before skilled FBI investigators arrived.
</p>
<p>
Time interviewed several interrogators who have worked with the U.S. military and retired intelligence service employees asking  &#8220;[what works?] How does an interrogator break down a hardened terrorist without using violence?&#8221;  Time reports &#8220;All agreed with Soufan: the best way to get intelligence from even the most recalcitrant subject is to apply the subtle arts of interrogation rather than the blunt instruments of torture.&#8221;

<p>
The <a href="http://www.time.com/time/magazine/article/0,9171,1901491-1,00.html">Time article</a> (3 pages) lists several examples of how smart interviewing succeeded. One example shows how these techniques work in a situation where time is of the essence.</p>

There&#8217;s been a lot of criticism of Obama&#8217;s plan to roll back the Bush tax cuts. Obama is proposing restoring the Clinton era rate of 39.6%.  Compare this to the 50% rate when Ronald Reagan was in office, 70% when Nixon was in office, and 91% when Dwight Eisenhower was president." width="480" height="360"/> <br /> Um yeah.  American really suffered under the tax rates during Eisenhower&#8217;s presidency.</p>

For an American the reader&#8217;s comments are very interesting.  The pro and con are not that different, to my eye, from similar discussions in the US. With one exception, in Ireland the debate is absent any discussion of constitutional rights.

In case you&#8217;re wondering, I&#8217;m not of Irish descent but I do check in with the <a href="http://www.irishtimes.com/">Irish Times</a> and the <a href="http://www.irishexaminer.com/">Irish Examiner</a> at least a couple of times a week.</p>

And so to all other peoples and governments who are watching today, from the grandest capitals to the small village where my father was born:  know that America is a friend of each nation and every man, woman, and child who seeks a future of peace and dignity, and that we are ready to lead once more.

We hear neighbors shouting and setting off fireworks.  McCain is giving a great concession speech.   My wife feared for the Bradley effect.  I was buoyed by optimism.  Yea!

<p>
There are many Americans who are disappointed, some deeply, with tonight&#8217;s results.  Since it has been a long time since Democrats, Progressives and, yes, Liberals have had won.  We need to remember our manners.  We need to remember the console of <b>Molly Ivins</b>.
</p>

<p>
I realize for many Democrats it has been so long since we won, we have completely forgotten the etiquette. And I realize I&#8217;m taking a chance here &#8212; there&#8217;s nothing more dangerous than overconfidence &#8212; but you have to practice for victory as well as defeat.
</p>
<p>
First rule: No gloating. Actually, there is gloating aloud, but only in the exclusive presence of other Democrats. Gloating in the face of Republicans is rude and unsportsmanlike, and just gives them one more thing to complain about.
</p>
<p>
Also, remember there is a possibility there may be some Republicans on the civil service staff &#8212; I have seen this when the R&#8217;s win &#8212; and it is really not good manners to watch them wailing around with their eyes brimming with tears.
</p>
<p>
Second, I&#8217;m sure we will all be full of grand theories if Republicans lose and we win. Dems will be ready to be helpful, offer advice and sort of try to perk the R&#8217;s up. I do not recommend this. It somehow never feels right to me when R&#8217;s are dumping truckloads of good advice on the D&#8217;s that they are, actually, sincere about it.
</p>
<p>
Third, celebratory jigs, reels and renditions of &#8220;Danny Boy&#8221; are best limited to Irish bars.
</p>
<p>
Fourth, try to refrain from insulting Republicans en masse. A good start would be, &#8220;You know, it was mostly the ones under indictment that hurt you.&#8221;
</p>
<p>
Now our real work begins.
